Please indicate this on the Registration. The cost will normally be $210 per additional question, but this cost may be less depending on the total number of questions and the survey programming required. See <a href=\"https:\/\/cusc-ccreu.ca\/wordpress\/?docs=cusc-procedures-manual\/appendicies\/appendix-additional-questions\" data-type=\"docs\" data-id=\"2152\" target=\"_blank\" rel=\"noreferrer noopener\">Appendix. Additional Questions<\/a> for more information and the template.<\/p>\n\n\n\n<p>Responses to additional questions will be provided in your institution\u2019s data files.
